4. Other income payments
5. Medical and health care payments paid to insurance companies, healthcare providers etc.
6. Crop insurance proceeds
7. Any fishing boat proceeds
8. Payments to an attorney
9. Cash payments for fish (or other aquatic life) you purchase from anyone engaged in the trade or business of catching fish;
10. Generally, the cash paid from a notional principal contract to an individual, partnership, or estate;

In addition, you are required to use this form if you made direct sales of at least $5,000 of consumer products to a buyer for resales anywhere other than a permanent retail establishment.
